Change of Status Report
This form must be submitted when a horse is gelded (change from stallion to gelding) or when it dies. |
Transfer Report
Please use the Transfer Report on the back side of the original Registration Certificate. If the registration certificate is older, and no transfer report is available on the back, please contact our office and an alternate Transfer Report form will be mailed to you. registrar@nfhr.com |
DNA Type Kit Order Form
This form is used to order one or more DNA Type Kits. The kits are order through the NFHR, however the pulled hair is not submitted to the NFHR. |
Microchip Implant Instructions
Instructins on how a microchip should be implanted. |
Notification of Microchipping
This form must be completed by the attending veterinarian when a horse is microchipped. |
Registration Application
This form must be submitted when a horse is being registered for the first time with the NFHR. |
Semen Transport Permit Application
The NFHR issues a permit for any stallion that is bred using artificial insemination and transported semen. This application must be completed and submitted with required fee before the use of transported semen will be recognized by the NFHR. |
Artificial Insemination Procedures
This describes the criteria that must be met for the NFHR to recognize a breeding by artificial insemination. |
Receipt for Transported Semen
This form must be completed by the stallion owner, the mare owner, and the attending veterinarian when a mare is bred using artificial insemination. |
